IRP Registration
To apply for a new IRP account, please provide the following documents:
- Copy of CDL or, if the applicant does not have a valid CDL, a copy of their DL and a copy of the driver’s CDL.
- Copy of Articles of Incorporation (if registering under a corporate name).
- SS# (for individuals) or EIN# (for Corporations or LLCs).
- Proof of Ownership (Title, form ST-556, or Bill of Sale from a dealer).
- HWY Use Tax (Form 2290) – if the vehicle was purchased more than 60 days ago.
- Proof of Address – Provide 4 documents with your name and current address, not older than 60 days. Acceptable documents include:
- CDL
- Phone, electrical, gas, water, cable, or internet bills
- Lease agreement for an apartment/house
- Mortgage statement
- Real estate tax bill
- Insurance (car or truck, must be a certificate, declaration page, or bill, not an ID card)
- Voter’s registration card
- Phone bill (mandatory)
